EXTENDED SUPPORT
Extended-Care facilities or support wing of our hospital offers wound care, IV therapy, and are typically able to accommodate more chronic medical health needs. Extended-Care facilities provide skilled therapy services within the home. Our Extended care domain represents a new level of care designed to provide skilled nursing services in a high-quality extended-care facility at less cost than in a hospital. For many patients, it will provide the necessary skilled nursing care to restore good health and enable the return to independent living in the community. For those with a continuing need for long-term nursing care, it will provide the early stages of such care.
Extended-care facilities offer both skilled therapy services (Physical Therapy, Occupational Therapy, and Speech Therapy) and long-term care.
Home Health Aides
Extended-Care facilities or support wing of our hospital can help the patient with his or her basic personal needs such as getting out of bed, walking, bathing, and dressing. Some aides have received specialized training to assist with more specialized care under the supervision of a nurse.
Home Assistant & Care Taker
While a patient is being medically cared for in the home, an assistant from extended-Care facilities or support wing of our hospital who helps with chores or tasks can maintain the household with meal preparation, laundry, grocery shopping, and other housekeeping items.
Companionship
Extended-Care facilities or support wing of our hospital help patients who are at home alone with a companion or assistant to provide emotional, mental comfort and medical supervision. Some companions may also perform household duties based upon request.
Care Taker
Extended-Care facilities or support wing of our hospital also help by arranging care takers to provide basic comfort to the patient through companionship, helping with personal care, providing transportation, emotional support, and/or helping with paperwork based on request.
